Alternative Residency Training Program

Alternative Residency Training Program

To apply, Alternative Residents need to complete the online Residency Enrollment Information Form (see link below) and upload completed documents below.

  1. Alternative Residency Checklist(for reference only, do not upload)
  2. Residency Training Program Plan
  3. Resident Registration Form – must be submitted within 30 days of starting residency; therefore, does not have to uploaded on initial application.
  4. CV (PDF format)
  5. Copy of Veterinary Diploma (PDF or JPG format)
  6. Cover letter

Note: Please have completed documents ready to upload before beginning the online form.

 

During their residencies, Alternative Residents also may need the following additional forms:

  1. Change of Resident Program Directors
  2. Resident Biannual Progress Report (due date of February 1st)
  3. Resident Annual Progress Report and Certification Statement (due date of August 1st)
  4. Alternative Residency MasterLog (to be uploaded annually with Resident Annual Progress Report)
    Note: This MasterLog file has been temporarily removed for updating.
  5. Resident Registration Form – must be submitted within 30 days of starting residency
  6. Signature pages for MasterLog and reports (can be uploaded separately if needed)
  7. Cover letter
